Topical Cream
Women use creams as a non-invasive way to get a dose of hormones through the skin. While estrogen and progesterone creams are generally easy to use, skin can sometimes be irritated by this method. We advise our patients to keep a close watch on their own body’s reaction to the medication. Applying a cream to the skin is called “transdermal” dosing, meaning that the medication is transported through the layers of the skin to arrive in the bloodstream. Dosing through the skin bypasses the liver and is generally considered to be easier on the body because of the lower dose needed. Application does take time and the amounts applied have to be precisely measured by the patient, so those looking for quick, effortless dosing might not prefer a cream.

Pills
Oral hormones are something you’re probably familiar with, since birth controls pills are typically composed of some combination of various forms of estrogen and progesterone. There are also progesterone-only pills. The doses of hormones in birth control pills aren’t targeted towards hormone replacement, so it’s better to use the expertise of a compounding pharmacy to arrive at a dose that’s perfect for BHRT purposes. Because an oral formulation has to go past the liver—which breaks down a large part of the hormones that pass through it—the dose of hormones has to be significantly higher.
